What does the marking 151K actually decode to?
151 is the standard three-digit ceramic code: 15 followed by one zero, which is 150 picofarads. The K is the tolerance letter for 10%. The 1KV on the second line is the working voltage rating printed by the manufacturer.
Is this a COG/NPO part or a high-K dielectric?
The part does not say, and we are not going to guess. There is no temperature-coefficient marking anywhere on the body. If your circuit is a filter, oscillator or anything where drift matters, treat the dielectric class as unconfirmed and verify against the datasheet before you commit the design.
Why are the leads bent partway down?
